To complete the EBOPRAS examination application form you will need:

When you have submitted all the information needed, you will receive an email with a copy of your application form
for you to print, and instructions on what to do next.

| Your passport which you must bring to the examination | |
| Dates: the date when you started training in Plastic Surgery and the date when you finished or will finish training | |

Your mobile phone number, including country code, in case of urgent need to contact you later | |
| A digital photograph of yourself. This should be a passport style photo - head & shoulders, in colour (not black and white) with a plain background, clearly focussed, and portrait orientation. An example is shown to explain what we need. Do not send a poor quality scan. You are all plastic surgeons and should understand what is an acceptable image. The photo must be in jpeg format, and should be 100kB to 1 MB in size. If the photo is inadequate, you will need to re-submit the entire application  | |

If you are a trainee, you will also need to download from the website
If you are a specialist, you will need to send a copy of your specialist diploma for the competent authority in your country
of specialisation. You will not need to download the logbook or assessment form.
